Deeply loved and adored husband of Gillian and dad of Jennifer, Harry, Hugh and Josh. On this, the first anniversary of John’s passing we would like to acknowledge and thank those who have helped and supported us throughout John’s illness and during this past year. We want to thank those who cared for John; the Oncology and Radiotherapy Teams at Cork University Hospital; the Radiotherapy Team at the Beacon Hospital Dublin; the Homecare Team of Marymount University Hospital; the Public Health Nursing team, Millstreet; all at Murphy’s Pharmacy Boherbue and the entire team at Áth Trasna Medical Centre, Newmarket. We owe a debt of gratitude to those who cared for John with boundless generosity of self, kindness and compassion. We would especially like to thank Ms Mag Allen, Oncology Advanced Nurse Practitioner CUH, Dr Richard Bambury, Medical Oncologist CUH, Ms Maureen Murphy PHN, Millstreet and our GP Dr Bertie Daly, Newmarket.  John was honoured to have known you and privileged to have been able to call each of you a colleague and a friend. We wish to thank Davy, Cora and Elaine of Tarrant’s Funeral Directors for their kindness and professionalism; the management and staff of Millstreet Community School for taking such good care of the children during John’s illness and over the past year; Fr Kevin O’Sullivan for the beautiful Mass to celebrate John’s life; John’s colleagues at HSE and Gillian’s colleagues at MTU for their unwavering support throughout John’s illness and over this last year; the children’s many friends for bringing joy and laughter back into their lives and into our home this past year; our many friends, close neighbours and family who have shown us such love and support through what has been the toughest of times. We are honoured that we have raised funds for Marymount University Hospital and CUH through a Gofundme campaign set up in John’s memory and we are eternally grateful to all who have donated. The fund will help establish a permanent Acute Oncology Unit in CUH. This is a fitting tribute to John and a most appropriate legacy recognising his commitment to healthcare improvement, his dedication to nursing and his strong links with CUH throughout his career.

John will continue to live on through us, to walk beside us, to carry us, to inspire and motivate us, he will forever be our hero and will remain the wind beneath our wings.

Anne, and the Family of the Late John Anthony O’Sullivan, Ballydaly would like to express our sincere and heartfelt gratitude to all those who helped and supported us during his illness, funeral and in the past 12 months.

The final days, were made so much more comfortable due to the professionalism and care of the following:  The District Nurses Maureen, Marie, Teresa and Sharon, Homehelp Christina, the doctors at Tullig Medical Centre, Staff of Mallow Hospital, Staff of Millstreet Hospital, Reens Pharmacy, Nightcare Tim, Staff of Marymount Hospice, Palliative Nurses Phil, Bina, Kathy and Eileen, Fr Jack and Fr. Seán. 

His passing was made so much easier by the sensitively of:  Daffodil Nurse Phil, South Doc & Davy Tarrant Undertaker. 

The Funeral Mass was exactly as John Anthony would have liked. This was down to the attention to detail and passion that Sharon in Parish office, Fr. Pat, Fr. Seán, Sacristan Julie and Singer Joan put into the ceremony. 

 We were so  grateful to Seán Radley that the ceremony was streamed to Millstreet Hospital for Anne. 

We were very humbled by the friends, neighbours and extended family that lined the road from Ballydaly to Millstreet when he left his home at 8.30am on that Friday morning.

And all those that stood outside the church and at the Graveyard where the final prayers were broadcast over his own public address speakers. 

We would like to sincerely thank everyone who wrote messages of sympathy on RIP.ie, sent Mass Cards and took the time to ring or text.  Your thoughtfulness was appreciated.

Covid-19 may have kept everyone physically apart, but we have never felt so surrounded by people with kind hearts and generous natures.

There were so many gestures of kindness and goodwill from so many people that we, as a family, appreciated and will never forget. We won’t name these individuals as there were so many, we’re afraid we’d forget someone. 

A Mass for all those mentioned (and those not), has been offered for your intentions,  in St. Patrick’s Church. Millstreet.

We acknowledge with gratitude the very kind support shown by our neighbours, friends and family who called with food to the house. Many thanks to the ambulance service and the staff of Cork University Hospital including the ODTI Team in Dublin.
Our sincere thanks to Canon John Fitzgerald and Fr Paddy Byrne who officiated at Jim’s funeral, and Julie and Sharon for their invaluable kind help. Thanks to Siobhán for the beautiful music and to Davy Tarrant for his assistance and professionalism. A very special thanks to Gordon and all Jim’s friends and comrades in the Army Reserve Defence Force who formed a guard of honour, along with the bearer party who helped carry Jim to the church and to his final resting place in St. Mary’s. As we lovingly remember Donagh on his first anniversary, we his family Mary, Denis, Jonathan, Noríde, Siobhán, Réiltín and Seán wish to express our sincere thanks to all who supported and sympathised with us on our very sad loss.

Thanks to all who attended the funeral, Mass and burial as well as to all who sent mass cards, letters of sympathy and floral tributes.

Our sincere thanks to Canon John Fitzgerald, Fr Seán Tucker, Fr Tom Leane and Fr Kieran O’Driscoll CUH who officiated at Donagh’s Funeral Mass.

To the kind neighbours, friends and relatives who brought food to the house.
